Sonic's reaction suggests that he's rarely ever like this.

As you can see in the picture, in its phase 2, it bursts arms out of its body filled with blood, and its eyes come out of its mouth filled with mucus — its OneWingedAngel form is somewhere between StarfishAlien and EldritchAbomination. Its head is a Venus flytrap filled with eyes, the whole thing enclosed in tongues with two huge ones hanging down.
