I was doing some thinking the other day, then it hit me. a werewolf must've broken into the cabin and killed the old couple, hence the vision telling Marie to run away and her vison of sister Ruth saying "don't hurt them". I am assuming it was the doctor because no other werewolf in the movie would have that type of strength. a tense scene just became a whole lot scarier. and worse, if the ending is anything to go by, Marie either got infected (likely) or was killed outright. and Drago still continues to exist. (shudders)
