In the end, Buggy has been left deeply paranoid and comes across as cowardly out of a refusal to punch above a certain weight class, while Shanks hasn't been fully sober in twenty years.
** The above combined with the fact the other former Roger Pirates hid that Roger had a son has left both Shanks and Buggy extremely bitter towards their former crew. Buggy more so than Shanks as he was locked in Impel Down for months without anyone even trying to help him but they all rallied to save Ace, whom most of them had never met.

** Ivankov's use of the Horm-Horm Fruit is widely loathed in general by any individuals with ''actual'' professional medical training, and it's established that either he doesn't know the first thing about real medicine and is literally PlayingWithSyringes, or he doesn't ''care'' about the medical code of ethics and is happy to literally gamble with peoples' lives by brute-forcing his hormone "cures" on them.
** In canon, Ivankov's GenderBender powers and his willingness to "aggressively recruit" Newkamas by transforming people against their will is played for laughs. In this fic, it's emphasized that Ivankov's ideology is incredibly creepy, and that forcibly transforming people from one sex to another can have massive psychological detriments to them.

** The Blackjack pirates spent nearly twenty years not knowing the fate of their captain.
** Buggy and Ace were in a HellholePrison. Their loved ones knew for sure that they would be subjected to physical torture and even possible sexual assault.
*** Adds a extra layer of this for Ace and Shanks once informed that Luffy had broken in Impel Down to rescue him since given Luffy's general naivete he might not even recognize a predator.

* MemeticBadass: Portgas D. Rouge continues to terrify despite being dead for twenty years.

* MyGodWhatHaveIDone: Roger was deeply apologetic towards Buggy and Shanks after the Edd War once he realized what it was like for them.

** First, breaking the pirate code is a huge deal among the older pirates and those who know about it, even if one isn't a pirate themselves. Despite finding it incredibly rude, they do excuse the ignorance of the younger generations due to never learning about it.
** One of them is there's a certain "proper" way to invite new crewmates on your ship. Unsurprisingly, that was Luffy's favorite one to learn.

* SuperDoc: The only reason Luffy survived Ivankov's so called "miracle cure" is due to Law's devil fruit. Not only does he have a medical degree but he knows exactly how to use it properly much to the relief of the other doctors who are sick and tired of Ivankov's ''appalling'' use of his devil fruit. That's actually how he first endears himself to Shanks so quickly after he saved Luffy's life is being a real ''competent'' doctor for once.
