Horror Under Tristram


  • Damage-Sponge Boss: The Butcher, likely the first endgame boss to be tackled by players, is a strong and relatively tanky melee fighter, but has no special tricks that need to be paid attention to.
  • Psycho Knife Nut: As expected, the Butcher fights with a bloody cleaver.

Infernal Machine


  • Counter-Attack: Once attacked it will automatically responds with mighty rune magic.
  • Mook Maker: The Infernal Machine's attacks are unremarkable; its threat mainly comes through an army of summoned elite monsters that fight for it.
  • Stationary Boss: The Infernal Machine can't move.

Death Projector


  • Helpful Mook: The Death Projector is normally invulnerable, but when it senses at least two (or three for the Uber version) attending officers nearby, it lowers its magitech shield and can be damaged and destroyed. Each officer has different skills, which pose different challenges; you should choose the officers which are easiest to deal with for your build.
  • One-Hit Kill: The Death Projector's Mind Flay deals enough damage to instantly kill you if you let it touch you.
  • Stationary Boss: The Death Projector has no means of locomotion.

Invasion


  • Shock and Awe: All of the Riftwalkers' attacks deal lightning damage.
  • Stationary Boss: Despite having legs, the Riftwalkers never move away from their position.

The Binding of Baal


  • Armor-Piercing Attack: The Horadric Mages are able to pierce up to 30% of your resistances, Tal Rasha up to 40%.
  • Elemental Powers: The Horadric Mages wield fire, ice and lightning powers, making it a good idea to stack resistance in this dungeon.
  • Shielded Core Boss: Tal Rasha is invulnerable until all of his Horadric Mage minions are dead.

Horazon's Sanctum


  • Thinking Up Portals: The Creature of Flame, although not very strong on its own, has the ability to open Dimensional Rifts, through which demons emerge until they're destroyed.

Akarat


  • King Mook: Akarat looks almost identical to his Followers, save for a white mist.

Assault on Mount Arreat


  • Kill Enemies to Open: Killing Narthal on the Arreat Summit opens a time portal to the Arreat Summit, where the player is transformed into Diablo and reenacts his battle against Bul-Kathos.
  • Shielded Core Boss: Bul-Kathos is invulnerable until all Shadow Wolf Warriors are dead.
  • The Turret Master: Shadow Wolf Warriors and Bul-Kathos can cast Guard Tower and Fortress, respectively, to summon invincible turrets that shoot arrows at you.

Lord Aldric Jitan


  • Death-Activated Superpower: Lord Aldric Jitan and his Rakkis Guardsmen cast Singularity when killed. Stay away from the dark vortex!
  • Elemental Weapon: The Rakkis Guardsmen deal Cold, Fire and Lightning damage with their melee attacks.
  • Shielded Core Boss: It's impossible to damage Lord Aldric Jitan before killing all Rakkis Guardsmen.
  • The Turret Master: Jitan and the Rakkis Guardsmen will summon guard towers which shoot you if you stand still for too long. Stay away from their range until their duration expires.

Inquisitor of the Triune


  • Teleport Interdiction: Town Portals are disabled in the Chamber of Offering until the Inquisitor of the Triune is defeated.

Bull Prince Rodeo


  • Animal Stampede: Rodeo can call upon a Stampeding Herd of exploding cows that trample all in their path. This ability is unblockable, and will kill you if you don't get out of the way in time.
  • Feathered Fiend: Rodeo can summon a wave of chickens that will pursue you relentlessly, inflicting devastating damage with their beaks.
  • Superior Successor: While the Cow King is a relatively standard super unique monster, Bull Prince Rodeo (who is described as his heir) is a full-fledged endgame boss.

Rathma Square


  • Reviving Enemy: While the Gore Crawlers can be attacked and killed, they'll automatically resurrect themselves after 10 seconds, and it is impossible to prevent this.

The Black Road


  • Flunky Boss: Buyard Cholik can summon a horde of Possessed Cultists to swarm you.
  • Playing with Fire: Buyard Cholik is a primarily fire based caster.
  • Tactical Suicide Boss: While Buyard Cholik is normally invulnerable, he can be damaged when he's standing in the fire circle in the middle of his room.

Legacy of Blood


  • Boss Room: Bartuc and his Blood Golems all reside in a very small room.
  • Tactical Suicide Boss: Bartuc, who normally is immune to all damage, becomes vulnerable after he uses the Wychwind ability, which temporarily reduces his resistances.

The Veiled Prophet


  • One-Hit Kill: The Veil Piercer's Death Spiral instantly kills anything it hits.
  • Stationary Boss: The Veil Piercer never moves away from the centre of the Chapel of Vanity.
  • Wolfpack Boss: The Prophet, the dungeon's main boss, is accompanied by two mini-bosses Gamuel and Oris.

Heart of Sin


  • Invincible Minor Minion: Azmodan is accompanied by Sons of Azmodan and Ravening Beasts, invulnerable and untargetable versions of Guardians and Firebloods.
  • Playing with Fire: Azmodan can cast Flamestrike, and his minions use Fire Fountain and Molten Boulder. High fire resistance will help against the latter, but the former remains very dangerous even against high fire resistance.

Judgement Day


  • Invincible Minor Minion: Imperius and Malthael's Holy Nephalem minions are invulnerable.
  • Time-Limit Boss: The Crystalline Flame Medallion can only be upgraded if you kill both Imperius and Malthael within 60 seconds.

Twin Seas


  • Death-Activated Superpower: Athulua's rat minions chase you down and cast Tremor on death. Kill them from a distance so as to not get hit by this spell.

Quov Tsin


  • Elemental Powers: Quov Tsin wields the elements, with his most devastating attack being a streaming blast of fire, cold and lightning bolts. Prepare for this battle by equipping resistance items.

Kingdom of Shadow


  • Invincible Minor Minion: The ghostly citizens are impossible to kill, but they are more than capable of killing you. Keep moving at all costs.
  • Zombie Apocalypse: The citizens of the city have been transformed into ghostly zombies that can never be killed. Run around like a damsel in a zombie film until you find the narrow ledge leading to the king.

Banisher of Light


  • King Mook: Destroying each Seal located in the peripheries of the Black Road releases a horde of elementals led by an Ultra Elemental,
  • Sequential Boss: Located at the centre of the Black Road at the beginning is the Pillar of Skulls. After it is defeated, Kabraxis will be released.
  • Stationary Boss: The Pillar of Skulls has no means of locomotion.

Toraja


  • Ability Required to Proceed: Edyrem minions, provided by the Crystalline Flame Medallion from Judgement Day, are the only things that can break the invulnerability of both the Symbol of Time (which must be destroyed to open the portal to Toraja) and every enemy in Toraja itself.

Eve of Destruction


  • Adaptational Badass: On Hell difficulty, Baal is treated as not just the Final Boss, but an endgame boss, and is much tougher than his vanilla counterpart as a result.

Lord of Lies


  • Self-Duplication: Belial's signature move is summoning clones which will keep him safe as he cowardly hides.

Ghosts of Old Bremmtown


  • Time-Limit Boss: Upgrading the Dragon Claw requires you to kill the Dark Star Dragon within 90 seconds of entering Bremmtown without dying.

Yshari Sanctum


  • Ability Required to Proceed: Each sector of the Yshari Sanctum is inhabited by a boss and various minibosses, which drop pieces of the Brotherhood of the Vizjerei and one of the Horadrim. In order to fight Valthek, you need to equip both sets, which provide the only abilities that can harm Valthek and his minions.
  • Helpful Mook: You can only use the skills from the Yshari Sanctum sets (Holybolt, Arcane Blast, Iron Shield) near Valthek's Mystic Aegis minions, which mean while you need to clear the summons, you must leave at least one alive, because you need Holybolt to kill Valthek.
  • Stuck Items: You are effectively forced to equip the Vizjerei and Horadrim sets for the Valthek fight. While in theory you are free to equip whichever items you like, without the skills granted by these sets you can't scratch neither Valthek nor his Mystic Aegis minions.

Xazax


  • Forced Transformation: Xazax can transform you into a defenceless maggot leaving you open to attacks, so be sure to keep your distance from his minions at all times.
  • Invincible Minor Minion: While Xazax's Penitent minions can take damage, their utterly ridiculous amount of HP means that you will not kill them.

Astrogha


  • Death-Activated Superpower: When destroyed, a Spider Statue will both rejuvenate Astrogha and unleash a curse that cripples the assailant. If left alone, Spider Statues will eventually collapse on their own.

The Void


  • Cast from Hit Points: The Soul of Wrath is immune to all damage, but all of its attacks also deplete its HP. Your job is to provide it with an outlet for its wrath until it kills itself, while keeping yourself alive since it restores all HP upon killing.
  • Death-Activated Superpower: The Soul of Sloth fires a barrage of projectiles at you after dying.
  • Mêlée à Trois: Inarius and Lilith, the two final bosses of the dungeon, attack each other as well as you.
  • Rare Random Drop: The Soul of Sloth has an extremely low chance of dropping the Scroll of Sloth, meaning that you'll have to kill it over and over again (it always respawns a few seconds after being killed) until the scroll drops.

Nephalem's Sacrifice


  • Boss Rush: Each of the Edyrem's Path's 10 dungeons bears a resemblance to an area that you should have already visited either in the main game or previous endgame quests, and is home to upgraded versions of bosses that you should have already fought (for example, the first area is a copy of the Burial Grounds, whose boss is Moreina the Corrupted, an upgraded version of Blood Raven).

Archbishop Lazarus


  • Creator Cameo: This boss is not the actual Lazarus character in the first Diablo, but is a reference to Brother Laz, the creator of the mod. His minions are called Groupies, the charm he drops is called 'Glorious Book of Median', and he actually used to be called Brother Laz until version 2017 removed all references to real life people. His official description reads as:
    Some even believe that the world as we know it was shaped by an individual human on another plane, one 'Archbishop Lazarus' with a calculation engine powered by lightning. According to this cult, when humanity complains too much about the difficult living conditions in Sanctuary and the scarcity of divine weaponry, he may send his avatar to the mortal plane to deal with those who annoy him in person...
  • The Turret Master: Lazarus can summon Black Hand Totems that kill you if you go near them, and render you brittle as glass and heal Archbishop Lazarus and his Groupies if you destroy them.

Herald of the End


  • Eldritch Abomination: Samael, Herald of the End, is an elder being that lurks beyond Sanctuary, the High Heavens and the Burning Hells alike, and has powers beyond this realm.
  • Time-Limit Boss: Samael only drops a trophy if he's defeated within 11-12 minutes of entering without dying.

    Rifts 

Island of the Sunless Sea


  • Invisible Monsters: The Sunless Revelers, who make up the majority of this rift's population, are normally invisible.
  • Mini-Boss: In place of an actual boss, the Island of the Sunless Sea is home to six Sunless Angels. Each drops an Angel Bag, and six Angel Bags combine together into the charm Six Angel Bag.

The Secret Cow Level


  • Composite Character: While the original Cow King from vanilla Diablo II was a mere super unique monster with no lore, this Cow King is described to be no one other than Wirt from the first game, who merged with a bull, crowned himself the Cow King, then proceeded to mould numerous cow herds into faithful and conscious subjects.

Torajan Jungles


  • Damage-Increasing Debuff: Followers of Akarat reduce the resistance of all nearby enemies when killed.
  • Fauns and Satyrs: The Khazra Clan are this dungeon's version of the fanatical axe throwing goatmen that are ubiquitous in the main game.
  • Helpful Mook: Followers of Akarat reduce the resistance of all nearby enemies when killed, making them essential in breaking the invulnerability shield of Akarat and allowing you to kill him.

Kurast 3000 BA


  • Field Power Effect: Shadowgate Totems emit a magical field that significantly reduces the damage taken by nearby enemies.

High Heavens


  • Fallen Angel: Ethereals are manifestations of lingering angelic spirits tainted and subverted into avatars of evil.
  • Invincible Minor Minion: Lightwells are completely untargetable and invulnerable and must be avoided.

Tran Athulua


  • Rain of Arrows: Tran Athulua is heavily guarded by bow-wielding Amazon warriors, whose sheer number of arrows may overwhelm a weaker hero.
  • When Trees Attack: Beware the Trees of Fire with their ability to cause devastating wildfires.

Teganze


  • Classical Elements Ensemble: Umbaru Spirits come in six variants, each corresponding to an element, that inflict damage based on their elemental alignment. Don't forget to equip your resistance items.
  • Elite Mooks: Umbaru Spirits in Teganze can spawn as champions (the only enemies that still use this mechanic from the vanilla game), which have improved stats compared to normal ones.

The Triune


  • Armor-Piercing Attack: Malic (an unkillable boss in the Bastion of the Triune) has the only skill which can harm Lucion (the main boss), who is otherwise immune to all damage.
  • Dual Boss: Defeating Lucion requires you to make him fight Malic, then survive both bosses' attacks as Malic kills Lucion.
  • Set a Mook to Kill a Mook: Lucion can only be killed by getting the Demonic Charm spell from a previous boss, charming Lucion and moving him to Malic, the only thing that can kill him.

Duncraig


  • Throw Down the Bomblet: Some Slain Souls wield powder kegs that they throw over large distances and then light the fuse, causing a devastating explosion.

Vizjun


  • Mecha-Mooks: Vizjun features melee robots, laser robots and machine-bolter robots led by a super-computer.
  • Nuke 'em: Vizjun's automated defences will deploy even nuclear MIRV strikes when you enter the protected zone.

Fauztinville


  • Pinball Projectile: Harpylisks attack with bouncing blades, making them high-priority targets for summoners.
