This could be for any TTRPG. This example is D&D specific. I was adventuring with a group of two other players and two NP Cs. We encountered a lich. Due to good planing and a delayed action my wizard wiped out the lich's minions in the first round of combat and the lich in the second. The entire time my wizard kept reminded everyone we have to find the Phaltory. The game session went on, everyone forgot about the lich. so we went back to town. somewhere between game sessions it hit me, we missed the Phaltory.
This can be applied to any TTRPG, your party has made it out of the dungeon or safely back to town and then it hits you, you missed something/ left/ forgot something and it will come back to haunt you and the rest of the party
