Jon getting a Game Over, not by dying to a boss or some hazard in a stage, but by falling through a tiny little crack in a raft in an adventure field.
Someone just added this instance to the page, however I don't think it should count because Jon ended up dying and getting a game over due to a glitch and poor programming and not because of lack of skill.
