A more obscure example: In Mass Effect 2, when you do Thane's loyalty mission, you see his son chase a politician into a room. You enter the room to see the politician just standing there; he then crouches and you see the son has him at gunpoint.

You see a character standing there, and everything seems fine at first. Then the character falls over and you realize he's dead, or he crouches down and you realize someone directly behind him has got a gun pointed at him.
This happened with the Hispanic guide at the beginning of Raiders of the Lost Ark. I think I've seen it in a few other places.
