Not necessarily, especially if the person who said it addresses again a second time and much more slowly for emphasis so that they know they heard. For example, in the remake of The Parent Trap (1998), there's this:
- Nicholas: What's going on?
Meredith: (raging) Here's what's going on, buddy: the day we get married is the day I ship those... brats off to Switzerland! Get the picture? It's me or them. Take your pick.
Nicholas: (gesturing towards his daughters) Them.
Meredith: (refusing to believe what she heard) Excuse me?!
Nicholas: T-H-E-M, them. (beat) Get the picture?

This is an idea for a trope when someone will ask, in a dangerous voice, "What?" or "What did you say?" or any other variation thereof. It can be a warning sign that their Berserk Button has been pushed.
