Private servers are too much bother. People who are more lazy than cheap will buy the game because of the added hardship.
Also, private servers tend to suck ass from what I've heard. Anyway, you lose most of the benefits of playing the game, like having an actual community.

Jonah FalconThe Cow level was a bad joke that mostly existed to give you shitloads of free loot and exp. Plus you had to beat the game to even access it. I never beat the game.
I reached act 3, burned out, and never played again.

I personally thought Diablo II was fun for a while. Torchlight was fun too. I can't comment on Titans quest having never played it. Borderlands however is of a completely different genre and brings up an apples and oranges discussion.

Complete pirateproofing is impossible. Wow also requires constant connection and just look at the amount of private servers. It's main effect is anti-cheating, but even then they could make a single player mode fully separate from online. That would have gotten them some more sales, but they didn't go for them.
Though, as long as somebody is pirating it, setting up a truly private server for oneself is also on the list of possibilities. It's only legitimate players who can't do that.
