Both Jack and Kirshner died on the operating table at the end - the movie shows them going to heaven and in hell.
- Kirshner is condemned to hell as a disembodied head, begging for help.
- Jack is happy and riding off (to heaven) with his friends and singing a gospel song. He doesn't seem to have any bad effects from the major surgery he just had.
