1* PromotedFanboy: Timelock Studio, the main developers behind ''Siberian Mayhem'', is entirely composed of well known modders in the Serious Sam community. Most of them were officially hired to be part of Croteam after the game was released.
2* RecycledSet: Most of the levels in Rome reuse assets from ''VideoGame/TheTalosPrinciple''. Meanwhile, a few guns and props come from ''VideoGame/HotDogsHorseshoesAndHandGrenades'' due to both games using the same assets.
3* RefittedForSequel:
4** During the game's early stages of development, Croteam wanted to implement puzzle sections to break up the action. However, the developers liked the puzzle sections so much that they shelved ''Serious Sam 4'' for the time being and began working on a puzzle focused game, which became ''VideoGame/TheTalosPrinciple''.
5** There were plans for an Octanian Soldier variant that throws grenades, as seen in one of the trailers, however it was cut from the final product, presumably because the inclusion of the Beheaded Bombers made them redundant. This enemy was later introduced as a new enemy type in ''Siberian Mayhem'' and was given a similar, yet distinct behavior.
6** There were also plans for a refinery level set between "The Package" and "From Earth With Love" in the final game, but it was scrapped due to time constraints. ''Siberian Mayhem'' later added it back.
7** Another ''Siberian Mayhem'' addition is the Crossbow, which was also cut from the main game.
8* ScheduleSlip: Development on the game started on April 2013. Then a feature intended for the successor of ''VideoGame/SeriousSam3BFE'' got turned into an entire, unrelated game, ''VideoGame/TheTalosPrinciple''. Then the series saw its first VR entries (''Last Hope'' and VR versions of ''[[VideoGame/SeriousSamTheFirstEncounter TFE]]'', ''[[VideoGame/SeriousSamTheSecondEncounter TSE]]'' and ''[[VideoGame/SeriousSam3BFE BFE]]''). The game was finally announced as ''Serious Sam 4: Planet Badass'' on April 2018 with a slated release date for August 2020. ''Then'' the subtitle was droped, and ''then'' it got delayed for September 2020.
9* WhatCouldHaveBeen:
10** At the time of its announcement, the game was originally slated as a Platform/GoogleStadia and PC exclusive; in order to compensate for this, versions for Platform/Playstation4 and Platform/XboxOne were released in 2021.
11** Originally the game was going to bring back floating pickups like the previous games, but after Devolver asked fans on Twitter if they wanted grounded pickups like ''VideoGame/SeriousSam3BFE'', the floating pickups were scrapped.
12** Sam was going to wield a machete instead of his combat knife, but the weapon was scrapped. It's still accessible in the game's files.
13** Four weapons ended up being cut from the final game. These include a Molotov Cocktail, the Flamethrower, and a Nitrogen Gun
14** The double-barrel shotgun was also meant to receive its own grenade launcher attachment, but it ended up cut.
15** The Octanian Raider ships were once meant to be enemies at one point in the game instead of only appearing during cutscenes, as indicated by leftover data in the game's files and a scrapped NETRICSA entry.
16** Beheaded Bombers were originally supposed to wear orange shirts, as seen in some of the trailers, while in the final product, they wear green shirts.
17** The game was originally going to be compatible with ''Serious Sam Fusion'', as it was the original plan to make it the official hub for existing and future games of the series, but the idea was shot down, possibly due to the game using a more updated version of the Serious Engine 4.
18** Early concept art for the game shows that [[BigBad Lord Achriman]] was originally planned to go OneWingedAngel into a draconic form, but this was scrapped due to time and budget constraints.
19** The game's FinalBoss, [[spoiler:Ugh-Zan VI]], initially had a significantly more demonic design with elements of a caterpillar.
20* WorkingTitle: The game had the subtitle "Planet Badass" when it was first teased in the E3 2018 teaser trailer.
