1* The game begins with an airplane crash, complete with the PlayerCharacter impaled by debris and pulling it out of their body. Leaving them to have to rapidly find a medical kit with which they can patch up an open, bleeding gut wound.
2** Hell, the entire evacuation is one. The military desperately trying to evacuate as many people as possible while also doing ''everything'' keeping the outbreak contained.
3* The F.L.E.S.H. system truly takes {{gorn}} to new heights, where you can kill the zombies in incredibly gory ways.
4* The Autophage. On the surface it seems like an average zombie plague, maybe even something that some MadScientist cooked up in their lab to set upon L.A for whatever reason. But under that bland surface and as certain truths come to light, the true scope of this sinister pathogen becomes frighteningly clear: [[spoiler: It's a genetic kill-switch, lurking within the very DNA of every last man, woman and child on Earth, and set to pop within the next hundred years or so, causing TheEndOfTheWorldAsWeKnowIt as every last living human alive falls victim to the Autophage and drives themselves to extinction in an orgy of carnage and destruction.]]
5** And the kicker? [[spoiler: ''It's a perfectly natural process''. This isn't the work of some artificial plague or genetic tampering, this is what Mother Nature created, built into us, and intended to be our extinction event. This isn't a simple ZombieApocalypse, but a full-blown CosmicHorrorStory.]]
6* The massive tumor-like growths of infected flesh found in certain places, voluminous enough in mass to cause ''earthquakes'' and seemingly sentient to a degree. And what's weirder and more horrifying is that no one, not even Reed and his group, knows what purpose it exists for, if it even has a purpose at all.
7* Ambient dialogue activated when a Burster is made aware of the player can have the zombie variant either begging for help from or apologizing to the Slayer. Implying the horrific possibility that the Bursters, and perhaps ''more'' zombies who cannot vocalize properly anymore, may still maintain some form of consciousness that forces them to watch themselves kill, maul, and devour other humans from within their own bodies.
8* One of the many actions the army and CDC were taking to curb the outbreak was to (and brace yourself for this one) [[spoiler: process large quantities of civilians and isolate them, gas them with chemicals to render them unconscious, and then liquefy them while they were still alive by pouring their bodies into a swimming pool converted into a vat of super-strong industrial alkali. The "meat soup" that resulted was then pumped into the sewers as a haphazard form of disposal, creating a [[MeatMoss nightmarish situation in the sewers]] that put even the nastiest dens of the first game harshly to shame.]] [[ShockingMoments Holy mother of god.]]
