3* The plot of ''High and Low'' in the manga. The president of a company had an aneurysm, and although Black Jack agreed to operate on him for 50 million yen, he had a rare blood type (RH-) which sent his men on a frantic search for a suitable donor. Fortunately, a laborer nearby was found to be the same blood type, and the operation went without a hitch. The two men bonded greatly over this event. One day, the laborer had an accident while the president was about to leave on a plane to meet a ''very'' important client. After a little internal conflict, the president decided to ditch the flight and give blood to save the laborer's life. Unfortunately, losing such an important client resulted in his company going bankrupt in short order. The laborer found the now-penniless president and told him not to lose heart, and that he will help the president in starting over. Black Jack then gave the men an envelope with a check of 49.9 million yen, as he "owed the president his change from the operation back then". The last panel showed the 2 men walking together, ready to start anew.
4* The chapter "Fake Doctor", where a kid gets another kid that he formerly bullied, who has parents that are actors, to play a fake Black Jack for his sick sister (who wants only Black Jack to diagnose/operate on her). Of course the real Black Jack shows up at the end and takes it from there, but it's adorable.
5* In “Vibrations” Black Jack has to operate on a pregnant woman, but the [[MeaningfulName vibrations]] from moving trains in a train station above are making it impossible for him to complete the operation. In desperation, the husband of the woman rushes to the station and begs the stationmaster to stop the trains for a bit. It must be noted this guy has been harassing the train station for a long time due to the noise bothering him, but now he comes in sincerely pleading to them to do something so that his wife and unborn child may be saved... [[ReasonableAuthorityFigure The stationmaster does not hesitate to stop the trains]].
6* One manga story involved a delusional man, bereaved over his dead family, pretending that a mom cat and her kittens were his family. When he snaps out of his delusion and decides to move on, he also abandons the cats, becoming incredibly annoyed with the tearful cats following them around. Finally, at the end, he decides to let the cats into his life after all, showing that he simply can accept that he can move on with the cats as part of his new family.
7* Episode 45 of the TV Series, "The Chronically Laughing Classmate" is a whole episode flashback of one of Kuro Hazama's inspirations to practice medicine. An AntiNihilist classmate taught him to start smiling again and later even saved his life. A reason Black Jack went into medicine was because the injury the classmate sustained ruined his vocal chords and made it hard to laugh... and Black Jack later was finally able to repay him by restoring his ability to laugh in an operation. Of course, it goes into TearJerker territory when the man dies from a serious case of pnuemonia, but the man still passes on in sincere joy laughing a grand laugh. The doctors note how sincere his laughter is.

9* Pinoko’s story of receiving an education has a much happier ending in the anime; after failing to complete college or fit in with elementary school, Black Jack arranges with the principal of the college to become Pinoko’s tutor, [[JerkWithAHeartOfGold which the normally uptight principal is happy to oblige in]]. Even better, some of Pinoko’s friends attend the tutoring too so that Pinoko doesn’t have to learn alone.
10* "The SL Called Life", which was supposed to be the final chapter, shows Jack dreaming about everyone he's met in his life. The dream ends on the closest thing to a confession from Jack to Pinoko, who appears to him in adult form. He remarks that she's been the best wife he could've had, despite her real appearance.
