Literally the whole episode is about Dean's fear of losing Sam, and the things he is willing to do to get him back. Even without the copious couple parallels, and regardless of a Ho Yay or platonic reading of this scene, it is undoubtedly about Sam. Trying to make it about Cas makes no sense.
