Physicians also have to follow a series of arcane rules to practice efficiently: procedures have to receive "prior authorization" by non-medical bureaucrats to be financed, and they have to be "in-network" (in partnership with the insurance company) to even be considered for payment (hence why "keeping your doctor" is such an important issue whenever the idea of a universal healthcare system is brought up). Only people who can afford the most expensive healthcare plans can access healthcare with any kind of ease.
** Hospital administration. Many hospitals in the United States are privately owned for-profit organizations. This leads to actual medical staff often being considered an afterthought and resources being stretched as far as they can go while money flows towards the top.

Publishers often rely on deeply predatory practices where money only flows in one direction: not only are they often prohibitively expensive, but they rely on unaccredited volunteer labor from researchers for peer review and require that scientists pay ''the journals'' in order to be published.
