Vee attempts to move operations into the more heavily trafficked library; later, upset at that yet more may be taken from her, Poussey gets drunk on her moonshine and attacks Vee.
Flashbacks concern the beginning of Piper and Alex's relationship. Alex's then-girlfriend Sylvia walks in on Piper and Alex's first sexual encounter and attacks Piper. After Piper approaches Alex at the bar where they met, Sylvia leaves a flaming bag of feces outside Piper's door when she's discussing the new relationship with Polly. In the present, Piper receives a letter from Alex, and subtext during a visit from Polly makes her realize that Polly was the friend Larry slept with. In retaliation, Piper has her new sister-in-law replicate the feces prank.

Afterward, she breaks down to Nicky and admits that she has mental problems. While Red is thinking it over, Nicky brings her the sample of heroin given to her by Vee's girls, turning it in before she can give in to her addiction.
