To say nothing of Runaan's horrifying screams - to put this in perspective, the fearless assassin, who's shown little to no emotion up until that moment had been reduced to screaming in terror. And it's pretty clear that Viren has done this to other people in the past...

This seems more along the lines of Fridge Horror. Sure, if you think about it too hard, it could seem off or slightly cruel. However, the moment isn't portrayed that way whatsoever. It's even explicitly stated that "She didn't need that leg to be happy, the world did."
