Kirk was facing court martial for the death of a crewman when video footage from the bridge shows him pushing the wrong button on his captain's chair. Spock discovered that the computer had been, in fact, tampered with, and the not-so-dead crewman had altered the footage. It's never explained why ''no one else'' (including Kirk's attorney) bothers to mention the possibility before the story's climax, however.

In reality, it's very difficult to fake these things digitally or otherwise. Any competent effects artist or Photoshop user will tell the difference quite easily.
