For what it's worth, the Paradox Interactive forums have banned the use of the word "kebab" to describe people, as well as all other uses of food in place of ethnic slurs.
Race-related Black Comedy is hard to do, yeah, unless of course you happen to be IN the group being insulted.
I recently started playing with fan-made civs in Civ 5 (still unable to play the sixth one), and just set up an amusing scenario: I installed a Russian map and populated it with nothing but various Russian leaders. I'm currently playing as Catherine versus Ivan the Terrible, Peter the Great, Alexander I, Nicholas II, Vladimir Lenin, Josef Stalin, Leonid Brezhnev, Vladimir Putin, plus non-Russian but geographically appropriate Kuchum Khan and Tygyn Darkhan.
This set-up has resulted in a lot of messages like "Russia has denounced Russia" and "Soviet Union has made a Declaration of Friendship with The Soviet Union".

In Civilization V when you played with a Modded Civ like Australia or someone else that had a City-State they would change the name of the City-State.

I tend to consider that Crosses the Line Twice humor is very hard to properly pull, especially whenever race, gender or sexuality are concerned, and that whenever it goes "global" a vast majority of its users will ditch the second line crossing and only keep it offensive.
I doubt Turkish - and, even moreso, muslim - people call themselves "kebab" as a symbol of national pride. If only because in several Western European countries kebabs are seen by the far right as a symbol of invasion by muslims.
Pinochet is a Nation-State, not a civ. And apparently Solano Lopez wasn't much better than him.
