Back on track-how much does The Postal Dude in Postal 2 qualify?
While it's true that the base game's tagline is "It's only as violent as you are!", in that you can technically do a Pacifist Run, they didn't load it up with Videogame Cruelty Potential and put you in dangerous situations for nothing...and then there's Apocalypse Weekend, where you HAVE to kill things and eventually nuke the whole city of Paradise, with the final line being "I regret nothing." Oh, and it's apparently a canon ending if Postal III is anything to go by (provided it releases, anyway). I'd say that bit just might make him qualify.

Alex Mercer keeps getting written off as such, but that's only due to fans playing him as such and not paying attention to the story at all.

I think the Alex Mercer everyone keeps thinking of isn't Blacklight/Zeus, but the REAL Alex Mercer. You know the one that makes the modified, super deadly version of the Redlight virus (Blacklight). Then after being taken out of the loop he steals a test tube of it and attempts to run, only to be cornered and in a last act of spit smash it in a busy sub way station. Resulting in who knows how many deaths in the events of the game. The real Alex Mercer was probably more of a Complete Monster then any other character in the game, even out of the actual mutant monsters. Even the Blacklight virus itself is disgusted by him and it eats people.

The best that developers and writers can do in this regard is to make a protagonist that can be interpreted as a Complete Monster. A CM is by definition impossible to sympathize with and making a PC who is unequivocally beyond sympathy would reduce your potential audience drastically even if the game gets past censors.
Caleb from the BLOOD series. A Satan-worshiping gunslinger who comes back from the dead, revenge being the only thing on his mind. Innocent and get in his way? He'll massacre you for shits and giggles. He doesn't give a fuck, you move you die. Hell BLOOD 2 opens with Caleb saying "It's Howdy Doodie time kiddies, 'the bad man's here...'". Complete Monster, no debate.
I am surprised that NONE of you guys mentioned the two main protagonists from both Man Hunt games.
There's a game in development from a small Finnish studio called Lucius. It takes place in the early 1970s and stars a kid born on the 6th of June 1966. Apparently, it will be based on committing crimes, unlocking demonic powers during the "levels", whithout compromising his façade of innocence.
What was that brutal Amiga side scroller where in one stage you take that chainsaw to a pregnant woman and it says something to the effect of "Better than an abortion!" in Finnish or Dutch or something?
It's not Damage: The Sadistic Butchering Of Humanity, though Mikael Modelcitizen does also count as a Complete Monster protagonist (AND IT'S SO MUCH FUN! :3).

How about GTA 3? I've yet to hear a single redeeming value for the main character there. Though I suppose its arguably Comedic Sociopathy rather than Complete Monster.

Forgot about that. What about that guy from Splatter House?
Rick from Splatterhouse is just a dude trying to save his girl (and his kid in 2), but I think the Terror Mask is evil. It powers him up, but never truly possesses him, and besides all you kill is clear evil anyway, he never has to make a choice between good and evil. So Rick is not a Complete Monster, we don't really know about the Terror Mask.
I've never played the 360 game though.
