I see what you’re getting at, but the suggestion doesn’t look like she’s transforming into anything and the pic on the NF page looks more startled than in pain.
Fine with the current. He looks distressed and it looks painful.
The suggestion just looks like her clothing is being thorn off her and she's reacting to that.
This trope is a bit hard to illustrate without making an overlap with some other trope, but I'm fine with the current, too. At this state, his transformation does not veer into Uncanny Valley strongly enough to be considered Body Horror. Also, it's clear from his expression that he's in pain. The OP does show the pain, too, but I don't see transformation in there.

I don't get pain from that just based on the visuals...creepy, but not painful.
That's a little better but still not terribly clear on the pain part, at least to me.
I think the context of the page itself helps 5.2 get the idea across that this is a depiction of a transformation. Moreover, I think the pain is communicated.
While I like 9, I think 5.2 does just as good a job while being more concise. So my vote's for 5.2.

The current image for Painful Transformation, while horrific, seems more Body Horror than anything else. I was thinking something like this◊ from Princess Tutu where Kraehe transforms might be better (and also cause I feel queasy looking at the current one XD;) The NightmareFuel.Akira page has a less visceral one that still gets the pain point across too. Maybe that one could be used
