As per the trope decscription:
Rounding out the archetypal Freudian Trio with The Spock and The McCoy, The Kirk must balance these opposing personalities while being able to take their advice and choose between them (or literally, choose "between them") without being overcome either by emotion or dispassionate logic, representing what in Freudian psychology is called the ego.
The image clearly shows Kirk between the other two, working as a middleman and giving both of them thumbs up. So it's not pure JAFAAC, at least it shows the "mediator" part of the trope, if not the "ego" part.
Looking at the The Spock and The McCoy pages too, I think only The McCoy is clearly JAFAAC, with the image providing no extra information. If it was an image of him treating the sick or something, that'd be a better illustration.

We need a pic of him arguing with Spock or Jim or just having an obvious emotional display for it to really be effective.

At any rate, we have the 7 votes required to change the image, so I went ahead and put 7.7 onto The McCoy - tagging the page, but keeping the old caption, for now, while we potentially further discuss it. Did crop out some dead space on the right; figure nobody will mind the tighter crop, there's still plenty of doctor stuff in the frame if that was desirable.
