I love The Dillinger Escape Plan's "When I Lost My Bet" but there's something about the realism of the gore and those...entrail-mouthed...things that is very disturbing and slightly nauseating.

Rock It by Herbie Hancock. Sure, it's tame compared to Aphex Twin or Nine Inch Nails, but you try watching that as a five year old and try not to shit a brick.
I remember being really afraid of the rogue car factory manipulator from the video for Chemical Brothers' "Believe" in my pre-teens. The fact that it could appear everywhere, even when it seemed physically impossible, made it all the scarier.

I don't remember much about the video, except there was a robot suicide bomber that replaced a singer at a live performance, and it was singing the song as it was built. To this day I cannot listen to the song, even without the video, without feeling overwhelming dread.
Also, The Wall movie by Pink Floyd. I don't know what my father was thinking, letting me watch it while I was not even 12, but I will never forget Pink's change from the rock star caterpillar to the neo-Nazi butterfly.
I dunno about music videos, but a ton of demos uploaded by this guy has one or two bits of horrific imagery that cause me to ask "How is this allowed on You Tube?!". Great synthesist, though. He can make the worst synths ever sound absolutely amazing, which can be quite deceiving.
