If we take the rational approach, Manic Pixie Dream Girl = Girl + Extroversion + Handful of quirks + Interest in a boring dude. I guess it's safe to assume the first 3 to exist, not sure about the 4th.

People are way more complex than just who they want to bone, quirks and extroversion. Hence why a MPDG can't exist, as their whole schtick is just being those four things. You can have traits reminiscent of them, but the concept of an actual real life MPDG is just as likely as your waifu coming out of the screen and starting a relationship with you.
