Hm.. you might look so wound-up as you're walking towards her, stiff steps, a blushing face. But this doesn't spell doom by itself — you're showing off your courage, and she'll notice it. Then you're mumbling something; your voice has crawled down your throat and it sounds like you're far away.
She's straining to hear you.

What might happen? It's not usually your fault. Most likely you'll get bumped off by the evil gears of synchronicity. Either some guy spills his drink on your pants, or her best friends start showing up, you are ruining my opportunity here. Or there's another guy, smart, conniving, and happens to also like her too. He pranks you.
And at the end, all you might make out with is a frown that'll last the rest of the day.
you get a case of word salad, or come off as a stalker with a crush, or just a plain stalker to her.
All three happened to the last two girls I liked.
example of word salad I had in real life at one kid, who turned it on me:
I said,
"Ya, you think it's funny now Corey, but you'll see who gets the laugh last"
him;
"Did you just say the laugh last?" he laughs at me.
You find them sitting together with a boy/girlfriend you didn't know they have.
They're really angry about something unrelated, and give you The Look when you approach them, destroying your confidence.

Those are some good things, but what are some of the less likely things that happen?
You mean you've never tried to approach someone in Real Life, or went over all of the possible things that could go wrong in your head to convince yourself not to approach the person in general.
As for less likely things, the post up above that mentioned accidentally touching her chest doesn't seem to happen outside of anime or manga.

Can we use "alien invasion" type unlikeliness?
If not, well...
They're drunk. They puke on you.
They're allergic to something you carry.
A friend shows up out of nowhere, makes a really embarassing joke about you and your crush.
A slug falls into your head, you panic and do stupid things.
Ah...
Where is this attempt at approaching someone happening? Outside? In the school hallways? A bar? An office? What would be unlikely for the setting? Think about it.

I agree, if you want something UNUSUAL, to happen, the environment would need to be known more fully. Also, the type of genre (general fiction, comedy, ect ect) would really determine what would be most appropriate in this circumstance.
Something that is absolved by the Rule of Funny, for example, would come into effect if this was a comedy.
You just can't get away with statistically unlikely scenarios as well in any other genre.

Yeah, you guys are right about that. Just so you know, the setting is a busy high school classroom, one where students are allowed to walk around and talk and stuff. And it's a comedy piece.
She screams and accuses you of trying to rape her. (My 13 year old brother has a classmate who does this with any boy she dislikes who comes within two feet of her.)
If I'm asking for advice on a story idea, don't tell me it can't be done.

accidental pervert? or He says something that is taken very sexual even though it wasn't meant that way, he trips and knocks over all her stuff, he tries to joke and comes off as an asshole.
the limits are limitless! The Rule of Funny pretty much means anything other than reality warping is acceptable as long as it's freaking hilarious.
