** ''Melee'' had one trophy received by obtaining every possible bonus you could at the end of a match. While about 90 percent or so were easy to get either through skill or sheer luck, there was one bonus called "Lethal Weapon" which involves you using a character's ''entire'' move set on an opponent ''without'' using consecutive moves, which means all of the basic moves, special moves, throws, and everything else in that character's arsenal had to connect with an opponent. This is especially difficult with using directional moves since the attacks have properties that restrict buffering additional attacks in weird ways, so you might do another forward attack when you try to do an upward attack, or something like that. The best part about this, is you have to connect all of these moves on ''one'' opponent ''without'' defeating them. Have fun trying this, and be proud if you ever achieve this bonus.

** The Steam release of ''VideoGame/SonicAdventure DX'' (in which achievements were added some time after the game was originally released) has "Metal Sonic" and "The Perfect Adventurer", both obtained by collecting all 130 emblems, and "Mission All Acomplished", obtained by completing all 60 missions. Even though there are less emblems, it's harder to collect all of them, as some of them are obtained in Adventure Fields (and there is an achievement for getting just those emblems). Plus, some missions can become insanely hard, requiring you to perform actions within Action Stages and not just in the Adventure Fields (and some missions can only be completed with a specific character).

* ''VideoGame/OneHundredPercentOrangeJuice'' has "Jackpot", which requires you to draw a particular card out of a 48-card deck that turns your d6 into a d8 (and you can only bring one copy of this card, meaning that even if all four players bring one, you still have less than a 10% chance of drawing it), and then roll a 7 three times in a row. Even discounting the odds of drawing the particular card you need, this has slightly less than a 1 in 500 chance of occuring (0.1953%, to be precise), and there is absolutely no way to force the dice to roll a 7. Earning this achievement is effectively like winning a real-life lottery.

** Lead God is difficult, but doable, especially with the addition of the Boss Rush mode in a recent patch. The achievement most worthy of 'That One' status as of this writing is ''Challenger,'' which requires completing the newly-added Challenge Mode. To explain, they took the already NintendoHard BulletHell roguelike and somehow ''made it'' '''''much''''' ''harder'' with the addition of room modifiers that range from being bombarded by missiles from offscreen, making certain enemies invincible until all the others are killed first, punishing missed shots by spawning in enemies, making the normally harmless debris and breakable pots shoot bullets at you when damaged, and a whole list of other hindrances. It's not so bad at first when you only have to deal with one per room, but - you guessed it - a room could be affected by ''up to three modifiers at once.'' [[OhCrap This includes boss rooms,]] and some challenges are ''tailor-made'' for bosses past the fourth chamber. The DiskOneFinalBoss also gets a string of new, faster attacks, '''all of which are guaranteed to deal a 5-second HPToOne effect''' regardless of your health because of another modifier, which also has a chance of showing up in sealed rooms on any floor. You might think that the game will allow you to test your luck skills for free, but the harsh truth is that you also have to pay ''6'' Hegemony credits every time you want to try again. For reference, you get about 2-3 on average from beating bosses[[note]]Luckily, after release of Advanced Dungeons and Dragons update it's possible to get up to ''10'' credits for killing bosses without taking damage.[[/note]], and 5 from beating the game.[[note]]After attempting 30 times, the game gives you a breather and reduces the price to one hegemony credit... but that's still 180 credits you have to fork over before the closest thing to a mercy mode kicks in, so it barely helps in the long run. What's worse, the game mockingly gives you Chaos Bullets after your 30th attempt, which you'd normaly get as a reward for completing challenge mode, somewhat rendering the whole mode useless, unless you are aiming for the achievement.[[/note]] All these things combined make it easy to see why this achievement is the 2nd least obtained by all players on Steam, [[http://steamcommunity.com/stats/311690/achievements/ sitting at a mere 1.7%]].

** The game has a nasty habit of relying on the RuleOfThree for some very notorious achievements, from ''The Power Of The Heart''[[note]]Kill three enemies with Cupid's Heart Bomb. Heart Bomb is a highly telegraphed ability with a small blast radius that most enemies will easily scatter and escape from with very few ways of keeping enemies in position to hit more than one. Despite Cupid being added to the game in the closed beta, ''The Power Of The Heart'' has one of the smallest completion percentages at only 1.2%.[[/note]] and ''Detonate'' [[note]] Kill three targets with a single use of Zeus' Detonate while his passive is fully charged on them, a feat which requires careful and precise usage of his other abilities and keeping a SquishyWizard in relatively close proximity to the enemy team for an extended period of time. (Completion percentage: 1.4%)[[/note]] to ''Fury Of The Desert'' [[note]]Finish off 3 enemies with a single use of Anhur's ultimate, an ability which tends to require focusing down a heavily damaged target just to get ''one'' kill. (Completion percentage: 1.3%)[[/note]] and ''Got Nuts'' [[note]]Stun 3 enemies with one use of Ratatoskr's Acorn Blast, which requires perfect positioning of a ShortRangeShotgun like ability with an extremely narrow cone. ** Other achievements require repetition of tasks that, while relatively simple to complete a few times in a match, are nearly impossible to accomplish repeatedly within the requested time period, like ''Bird Bomb'' and ''Bringing Down The Law'' [[note]]Kill 15 enemies with Odin's shield + leap combo and Tyr's ultimate in a single match, respectively (1.2% and 1.1% completion percentage respectively)[[/note]], ''The Crowd Cheers!'' Radiant Glory heals Achilles for each enemy God or minion he hits with an ability, and his abilities have decent [=AOE=]s to take advantage of this, with one of his abilities being capable of being cast twice before going on cooldown. The problem is that Radiant Glory can only heal a maximum of 4 times per ability, and the aforementioned second cast can only be used if the ability hits a god, which heavily limits the amount of opportunities a player has to go for the achievement. Particularly notable is that while ''The Crowd Cheers'' is not the achievement with the lowest completion percentage, all of the achievements with lower completion percentages are from gods who were released considerably later than Achilles, with the earliest one added to the game nearly a year and a half later. (Completion percentage: 1.1%)[[/note]], or ''Shattered Mind''[[note]]Fully stack ''Sanity Break''. How ''Sanity Break'' works is if Cthulhu can hit an enemy god 4 times (with a 5 second time limit between each hit) with either abilities or his third auto attack in his chain, then land a blow with ''Sanity Break'' within 20 seconds, ''Sanity Break'' gains a stack. The problem is that fights that last long enough to let Cthulhu build up enough hits to stack his passive without dying or the enemy escaping just don't occur often enough to get the twenty stacks needed to get the achievement. (Completion percentage: 0.2%. Note that Danzaburou, who was the god released after Cthulhu, has 1.3% and 1.6% completion percentage for his achievements.)[[/note]]. Not helping is that at the time of writing this, Smite's meta encourages shorter matches, giving players even less time to get these achievements.

**"Key to the City" requires you to complete the game at[[HundredPercentCompletion 100%]]. The game has 200 flying rats which can be tedious and time consuming which comes to the achievement/trophy "Endangered Species".
**"Wanted" requires you to reach the highest rank (10) by obtaining 5 millions. This requires lots and lots of grinding.
**"Auf Wiedersehen Petrovic" is by far the worst achievement/trophy to obtain. To do so you must '''win''' (not simply participating '''win''', is the key for the achievement/trophy) '''in everything'''. This means you must win in races in both normal and GTA, win the Cops and Crooks modes "All for One" and "One for All" being both a cop and a crook and beat the time target in every co-op mission. Attempting to obtain this achievement/trophy with public lobbies is impossible as many of them are god-like skills that defeats you multiple times. It is extremely recommened to have any person who help to you with this achievement/trophy.
**"Gooble Gooble" requires you to perform three strikes in a row in bowling. It requires lot of patience because many occasions you can't perform strikes. You must perform strikes in a row to count. Spares in a row won't count.
*''VideoGame/GrandTheftAutoOnline'' has the "Criminal Mastermind" award. To do so you must complete every original heist[[note]]The Flecca Job, Prison Break, The Humane Labs Raid, Series A Funding and The Pacific Standard Job[[/note]] with the same players, with the hardest difficulty and without anyone dying once. If someone or you dies even in Pacific Standard finale it's go back to Flecca Job!.

** The hero-specific achievements range from simple stuff, like attaching four Pulse Bombs for Tracer, to the ridiculous, like... well, read on. Most of the heroes have one achievement related to their Ultimate (for many Damage and some Tank characters, get four kills with one Ultimate use) and one related to another ability.[[note]] Most high-level players who mainly play a particular character will say that if you know how the character works, you can get both achievements without even consciously trying, but even then, there are exceptions. As for the four kill achievements, keep in mind that many of them amount to four "eliminations", not killing blows; as long as an enemy takes damage from the Ultimate and doesn't heal before reaching 0 HP, it counts toward the achievement.[[/note]]
*** The time between activation and detonation for D.Va's Self-Destruct can make "Game Over", which requires getting four kills with one Self-Destruct, a real pain, as most players with any game sense will know to avoid bunching up or congregating in open spaces if the enemy team has a D.Va, so unless they're all unconscious from an Earthshatter or stuck in a Graviton Surge, most of them will almost certainly get to cover in time.
*** Orisa's "Halt State" achievement requires that the player drag enemies into 1000 HP worth of damage with one use of Halt!; this effectively requires that Orisa drag at least four enemy characters, including one Tank (or three characters including two Tanks), into a high-damage Ultimate like RIP-Tire, Barrage, Dragonstrike, or Self-Destruct, but even then, getting the affected enemy players to take 1000 damage feels like a LuckBasedMission.
*** Following a re-work of Sigma's Gravitic Flux that makes it possible to cancel it by stunning or killing him mid-animation, the "Event Horizon" achievement, which entails getting three killing blows with one Gravitic Flux, requires a massive amount of luck and poor positioning by the enemy team to grab three players who are below half health and slam them against the ground without getting hit by a crowd control move.
*** Winston's "Mine Sweeper" requires clearing ten turrets or traps without dying. This almost requires either clearing most of an enemy Wrecking Ball's Minefield single-handedly (the mines count toward the total, but there are only fifteen of them, so even if you survive a follow-up Piledriver, you may be left with fewer than ten mines when the dust settles) or having a very persistent Junkrat, Symmetra, and/or Torbjorn on the other team who refuses to swap to a character who isn't as effectively countered by Winston.
*** Doomfist gets the dubious honor of having the perhaps most difficult achievement to date. "Cratered" requires the player to hit six enemies with a single use of his ultimate. Having the entire enemy team close together is a rarity by itself but his Meteor Strike is one of the slowest to execute, giving the enemy team plenty of time to scatter even before he has chosen his target area. Zarya's Graviton Surge is almost a requirement to achieve this and even that is far from a guarantee.
*** Genji is difficult enough to get the hang of without having to resort to meme-level spamming of "I need healing!" for most players, but then there's "Their Own Worst Enemy", which requires getting two killing blows with one Deflect. Your best bet is to hope that you have the millisecond-perfect timing and positioning to reflect an enemy Cassidy's Deadeye or an enemy Pharah's Barrage while the character in question is standing near several of their allies.
*** Junkrat's "Mine Like a Steel Trap" involves sending an enemy flying with a Concussion Mine so that they land in a Steel Trap. It requires either judicious arrangement of the two items in an enclosed passage and trying to bait an enemy player into walking into the traps, or hoping that when you set off a Concussion Mine, you just happen to blast the person standing near it in the right direction to land in the Steel Trap (if you're ''really'' quick and Steel Trap is off cooldown, you can throw it on their landing spot just before they hit the ground, but it requires very sharp reflexes).
*** Reaper's "Waste Not, Want Not" achievement requires the player to get three solo kills in ''one clip''. In other words, the player needs to kill three full-health enemies with only eight shots, which almost requires that all three enemies be 200 HP squishies (or, better yet, a 150 HP Tracer) and that every shot be a headshot, preferably at point blank range. Made even harder with the patch in May 2017, which made it so Reaper automatically reloads once he enters Wraith Form. Getting three solo kills with Death Blossom doesn't grant this achievement, although damaging the enemy and ''then'' using it will count, but finding three full health enemies and trapping them in Death Blossom after firing three shots is no easy task, either.
*** Soldier: 76's "Target Rich Environment" involves getting four kills with one Tactical Visor; even with the aid of a Nano Boost and/or Amplification Matrix, it can be immensely frustrating trying to get four killable enemies in view for long enough to reduce them all to zero health.
*** Sombra's "Power Outage" requires you to hack 6 enemies at once. In other words, the entire enemy team to be bunched up enough to hack them all at once, which is quite rare unless they are all sitting on the payload or the point (and most teams have the savvy not to do that if they know the enemy Sombra is close to an EMP). Her other achievement, "Hack the Planet", isn't much more forgiving, given that it requires you to hack 15 enemies without dying, which will probably mean regular translocating to safety in a way that shifts most of the burden of attacking or defending to your increasingly impatient teammates.
*** Symmetra's "Huge Success" requires her to teleport ''20'' players in a single game. While her 2018 rework made it possible to achieve this by putting down a portal and then having Sym herself teleport back and forth until the achievement pops,[[note]] Or, if you'd rather not go the LoopholeAbuse route, putting a teleporter pad close to spawn on a Control Map to make it easy for you and your teammates to get back to the point quickly.[[/note]] this was not always so; in its original design, each Teleporter could teleport up to 6 people (ignoring if it gets prematurely destroyed), so you were ''expected'' not only to have a team incompetent enough to constantly die in order to respawn and use it at all, but also to be perfectly able to deal enough damage in order to build up ''at least four Teleporters in a single game.'' The LoopholeAbuse of Sym herself using the portal twenty times was still possible, but still required that she build up to her Ultimate four times, and wasting those on teleporting yourself back and forth was a good way to enrage your teammates.
*** Widowmaker's "Smooth as Silk" requires you to get a scoped headshot while airborne; routine enough for mid-to-high-level Widowmakers (who might do this several times in a single game), not so much for low-level players. She automatically unscopes when she jumps or is flung into the air. That being said, all the game looks for is that Widowmaker is off the ground when she gets a headshot, which means that immediately jumping after getting a scoped headshot can get you the achievement. It's telling that, when a thread was created on the game's forum that asked "Which achievement did you get completely by accident?", the overwhelming answer was this one.
*** Baptiste's "Window of Opportunity" is almost completely beyond all but the highest level players, as it requires amplifying 2500 combined damage and healing with Amplification Matrix without dying. And that's ''amplified'' damage and healing, which means a total of 5000 damage and healing must pass through Amplification Matrix - and if the shots don't hit anything or the healing hits an ally at full HP, it counts for nothing. Fire Ultimates through the window? Most of them aren't damage boosted by it (notable exceptions include Tactical Visor). And while most Baptiste players can build up to an Amplification Matrix quickly, they still need to stay alive throughout the process.
*** Lúcio has perhaps the worst achievement in the entire game. "The Floor is Lava" requires him to get three killing blows while wall-running without dying. Thus, the situation requires Lúcio to be wall-running with a relatively weak enemy in his line-of-sight, and if a single teammate decides to help out and kill the enemy for you (as their job as damage-dealers entails), then good luck. At least you can push an opponent off the edge of the map and wall run as the kill is registered. If you're lucky enough to have a good Orisa on your team, she can also Halt! three players into the air so that Lúcio can knock them into a bottomless pit before they land, but it still requires razor sharp timing.
*** Mercy's "Huge Rez" achievement was changed from "rezzing four teammates at once" to "Rezzing six teammates without dying". Not impossible to achieve, but since Mercy has always been the [[ShootTheMedicFirst focal target since the start of the game]], her new skills force her to be in the line of fire much more often now.
*** Zenyatta's "Rapid Discord" achievement requires four people killed with Orb of Discord applied at the time of their death. The final blow can either be from Zenyatta or a teammate. Sounds easy enough, right? Well, it has to be done within ''six seconds'', meaning that if you aren't communicating with your teammates, getting this achievement almost requires the stars to align. Combine that with that fact the Zenyatta uses projectiles rather than hitscan, this means that people will likely fall just one kill short or 1 second short of getting this achievement.
** Most of the general achievements are pretty simple, from winning a match, to reaching level 50. However, a couple of achievements weren't popping correctly, with one requiring you to win a capture game, on defense, with no points lost. The achievement requires the capture bar to be at zero as the match ends, which can be very luck-based, depending on how good you are at defense, along with how well the enemy team is doing. Another, is getting 50 unlocks for a single hero, most of which, you'll get from RNG-based lootboxes.

The smallest screw-up from any player on Legendary difficulty is almost guaranteed to end the game. To emphasize its difficulty, a team of pros were able to earn the achievement with ''only 1 second left on the clock''. At the end of the event, when the mode's metrics were posted on the Overwatch website, ''fewer than one percent of attempts on Legendary difficulty'' ended in victory.

** The speedrunning achievements are easier than they seem because you can do them on new game plus, thus being able to just rush to the bosses. Bunny Extinction True Boss Rush definitely qualifies though. Besides that, there are two more achievements that stand out in terms of difficulty:

The level requirement means you can't cheese it by using low difficulties, and Rumi's fight is not only hard, but very long.
