Also, if Maki is anti-Kaguya, then it only makes sense Mikado is anti-Shirogane. So both of them sharing that unrequited aspect makes thematic sense

It seems more like Akasaka either decided that Mikado was in love with Kaguya later or was too preoccupied with making Mikado a loser in his introduction.

Also, Mikado is the Emperor in the Tale of the Bamboo Cutter that Kaguya is subverting. So him being the perfect suitor that ended up not getting the girl fits right in. Hey, remember when Prez, Mikado and Tsubasa had that whole talk about Prez and Kaguya's first time? Bet that was fun for Mikado.
