- If the Barkeep had the scars from the gender reassignment surgery, then John had them. Why didn't Jane see them and ask about them? What did John say about them?
- It's possible John and Jane only had sex once in a darkened room so maybe she didn't get the opportunity to see them.
- It's also possible that Jane wasn't too focused on them, or that the memories passed over time (overshadowed by all the other trauma she goes through). It's the same reason she doesn't realize that John's face isn't the same of her former lover when she finally sees herself in the mirror.
