* The tribal slaves, they are so broken it hurts. Also worth remembering it has been only a generation top since the bombs fell and some are already indoctrinated into being slave forever. Sure the Church members, Children of Forest and Metro citizens all have their despairs and fanaticism in some way or other but the slaves gave up all their self esteem and are willing to die for their masters' amusement.

* The backstory for the Pioneers and the Pirates on the Taiga level, they were all originally students of a teacher who taught them survival skills and initially they all got along well and seemed capable, so the teacher left them to their own devices, only to be horrified when the pirates formed and became just as bad as the bandits they were fighting against, the final tape of the teacher is especially heartbreaking to listen to as he despairs at how the pirates seem to outnumber the pioneers and he commits suicide out of guilt and despair for how terrible the world has become.

* The Dead City, all of it, especially when you find out that the many citizens died because the higher-ups hoarded most of the anti-radiation medicine all for themselves and had anyone that rioted in protest executed and dumped into a pit, the letters you read paint an especially heartwrenching picture, especially the one from a security guard who watched the citizens overrun a military checkpoint and saw the bombs falls. To top it all off, [[spoiler: Miller dies of radiation poisoning saving you at the end of the level, and of the entire game, in the good ending. In the bad ending, Artyom dies, too]].

** It gets worse in the Two Colonels DLC, where [[HopeSpot at first it seems as though the hoarding is to build up enough to get everyone out]] and the riots ruined it, only for it to be revealed that the higher-ups never intended to evacuate the civilians and only cared about their own escape.

* Sam's story in general. He's a [[SemperFi US Marine]] from the embassy guard detail, who was very nearly lynched in the immediate aftermath of the nuclear war by survivors who saw him as the only "enemy" available to take out their anger and grief. Being a ShellShockedVeteran of Iraq, he didn't even try to resist the lynch mob, as most of them were women and children, and [[WouldntHurtAChild he couldn't bear the thought of even so much raising his fists against them]]. Only Miller's [[BigDamnHeroes timely intervention]] saved his life, and earned Miller Sam's UndyingLoyalty. Sam became one of the first Spartan Rangers, and spent the next two decades fighting alongside Miller to protect the people of the Metro from monster and maniac alike, becoming one of the Colonel's personal retinue. As far as Sam knew, he might well be [[LastOfHisKind the last American left on Earth]], and Miller and the Spartans are all he has left. He missed out on the Battle of D6 on account of being in the hospital with life-threatening wounds at the time, and as a result blames himself for Miller's loss of limbs and the deaths of Ulman and so many other Rangers. As the Aurora heads out into the unknown, [[HopeSpot he now has some hope that there might be survivors in the US, including his family]], but at the same time is constantly heckled by jokes from his fellow Rangers about which side he'll be on if they should encounter the NATO occupation force that Miller seems certain is just around every corner. The thought of encountering other Americans who might potentially be hostile has him in turmoil, but he really doesn’t appreciate his FireForgedFriends questioning his loyalty, no matter how light-heartedly. To top it all off, Artyom's diary reveals that Miller's [[WindmillCrusader paranoid insistence]] [[ImStandingRightHere that there are hostile American occupation forces behind every blade of grass]] has convinced Sam that the Colonel doesn't trust him, despite twenty-two years of loyal service. Miller himself never says anything about it; he's too focused on his crusade against the phantom invaders to give a moment's consideration to Sam's state of mind.

** The Admiral, one of the Pirates who's set up shop in a nearby lumber mill--which, by the way, is the most irradiated part of the Taiga level so far--is built up as being the worst of the bunch, so extreme that the other Pirates don't want anything to do with him and who you'll probably have to kill in order to continue on to the dam. He reacts to Artyom shooting at the shrimps around his base with great annoyance because it attracts their attention, threatening to send his fighters down to kill him. After you restart the mill's electricity and take the elevator up to the second floor, you enter the room he's in and find that the Admiral is a broken, insane man stuck in a wheelchair from a spinal injury, and his goons--all two of them--are both dried-up corpses that he does the voices for as he copes with his loneliness. Suddenly, you don't want to hurt this guy and will probably just want to sit down and humor his mad ramblings until he falls asleep right there before moving along. And, with the forthcoming revelation that the valley is doomed in mind, probably no one is going to go there and help him....

* Entering a certain house in Volga has you find a large room filled with corpses. Continuing on, you find a body hanging from a noose. The nearby journal is that of a doctor trying to treat irradiated patients, slipping into despair as they die one by one. When he woke up and discovered he was the only one left alive, [[DrivenToSuicide there was only one thing he could do]]. The house is filled with ghosts when you turn to leave.

** Also for the fact that the valley they all live in is doomed as the dam is about to crack, and with the water being radioactive, the dam breaking would wipe everyone in the area out. It's sad for the Order as they had high hopes of settling down there, and it's more gut-wrenching when you read the unfinished letter from one of the members of the Pioneers, despairing that their expedition to find the reservoir turned out to be all for nothing.

* Both the endings, too. The bad for obvious reasons, the good for more subtle ones. Hearing Miller talk about his regrets and everything he forgot in his desire to keep his position is surprisingly affecting.

* The sheer scope of the mass grave you find in the early game. Made worse when you hear on the radio that survivors in Vyshny Voloychok sent an expedition to Moscow, who haven't returned. [[FridgeHorror The implications are clear.]] Made worse when a radio personality from said city drives home how utterly crappy Moscow is even from an outside perspective.
* One of the most notable is during the Volga level, where you can, if you've done the right sidequests, enter a room filled with coffins, where the local fanatics have been laid to rest. Some of those coffins are very small...
* Hearing Kirill try and contact his dad on the radio (listenable during the train level before Novosibrisk). It starts with what sounds like a more professional check in, with Kirill using codenames, stating that his father has missed 12 communication windows. Then Kirill calls him by rank, asking for a response and his voice starting to crack. Last he is full on sobbing, begging for his dad to respond and come back to him. When you meet him later, he states his dad had been out of contact for a month. You start to realize that Kirill knew full well that his father had most likely been eaten by mutants, but kept up the act of soldiering, radio contact, pretending that his father was just still on his mission, all as a coping mechanism.
