I remember this happening in an anime once. For a while, I didn't know what it was called, but I found the name recently: Deltora Quest. I think the hero is fighting one of the main bad guys, and he aims for his heart, which is located on the right side of his chest since he's inhuman. The Hero was slightly off, and the bad guy makes a comment similar to, "Oh, I see you were aiming for my heart." Is this it?

Something like this also happens in Serenity between Mal and the Operative.
There is a scene in this tv show or movie where someone shots or stabs someone and the person who shot someone intially misses any vital organs, causing the person to live. I believe the shooter or stabbed had to shoot or stab the victim because someone had to believe the victim was dead, when the victim isn't? I believe, I'm not really sure... and I think the stabber or shooter says, "Oops, I missed."... I don't know...
