Surprised by how this one turned out. Death still not obvious enough?

Yup. Looks like she's smiling or trying to, and some women bleed a lot during birth without dying. Plus I think it's technically considered a spoiler (the episode aired last year, but it's considered a big twist)?

How about the death of the Biblical Rachel, cropped to show the baby, the childbirth bed, and the people crying?

Going to go with 3 on this one. It's a little cluttered with many people, but the emotions are obvious, and even with that, the baby is still clear.
