As Disambig pages are in Main/ like tropes, you'll have to request one here.
To make a trope subpage for a work or a media subpage for a trope, if it's not in the "Create New" menu, you have to edit the URL. The first word after "https://tvtropes.org/pmwiki/pmwiki.php/" is the Namespace, the second is the page title. Replace the namespace with the trope name while the last part of the URL remains the same as the page is a subpage of. You'll likely see a blank page asking if you've meant a pre-existing page, which you can edit and add content.
Any other pages not in Main/ can be made with above method.

I want to create a sub-trope of "Lampshade Hanging" (to be called "Overplay Gloss"). How do I create the page?

Only the first post can be "stickied" (stuck to the top of each page on the thread) but it can be either (1) edited to include a link to the post or (2) have its content replaced outright with the original contents in a folder. I prefer (2).
