Given how much of an Omnipresent Trope this has become, I'm not convinced there's a point to having examples here.
(edit) aside from that, some of the examples belong under Bragging Rights Award or Self-Imposed Challenge instead (e.g. Net Hack, where "100%" is defined as "have the score counter roll over MAXINT").

Rhetorical, eh? ... Eight!I think that it's not as Omni Present as you seem to think it is especially with so many games like The Sims, or Oblivion that are so open ended. There is no reason to cut examples. This certainly isn't the largest example page we have and it very much lacks more than a fraction of the games we trope. I think we can just sort it.

Some games like Persona 3 got rereleased with a reward where one didn't exist before.
Hell only Final Fantasy X 2 was the first Final Fantasy game to have this (the FFV one is very very borderline and the Final Fantasy IX is misuse I think.)
Now with X Box cheesements and such it might becoming common but its not there yet.
Just sort by Game genre then non-video games at the bottom IMO.

- Games with a "% completion" counter (regardless of whether it goes to exactly 100%). Examples thereof tend to overlap with Last Lousy Point.
- Games with one or more major sidequest like "collecting all the 50 trinkets" that has no real bearing on the completion of the game, but is a form of artificially increased playtime. These are almost always a Cosmetic Award or Bragging Rights Award.
- Achievements / Medals, which tend to be random tasks unrelated to one another, which again tend to have no bearing on actual completion of the game. These are almost without exception Cosmetic Award.
- Self-Imposed Challenge, when the game itself makes no mention of the "100%" task you're supposed to be doing. Actually I think all Self-Imposed Challenge examples don't belong on this page.
So a soft split would also be a good way of organizing this.

The way I see it, Achievements in general aren't 100% Completion, but a specific "completed everything" Achievement could be.
Ok, so we have all the tropes we need, we just need to clean up this page and sort them into the right tropes. Then list the common ways of filling the completion meter with links to those trope saying that if they're just examples of those things without the trope, post them there.

Unless we just want to make that a different trope. Like "Game Designers Fail Fractions Forever" or something.
I think the issue of games crawling over 100% is actually some variety of Rank Inflation (just as ranks suddenly needed to surpass A+, percentages had to climb above 100%). If it doesn't fall under Rank Inflation, at the very least it's a sister trope.
Reminder: Offscreen Villainy does not count towards Complete Monster.Regardless of the Rank Inflation, if there's an absolute maximum you can get, it counts, whether they label it 100%, 125%, "S", or just 10000/10000 possible points. The point is that there must be a counter somewhere.

Right now, the examples section, comprised as it is of one long Video Games section is one big chaotic mess and it's impossible to find anything specific without some effort.
Since this trope is (largely?) video game specific, I think sorting the examples by video game genre would work best (see, for example, RPG Elements for a page which has done this).
