Case 03: True Cannibal Boy is a 2023 RPG Maker MZ Visual Novel developed by Sounding Stone and MaouCat Studio as part of the Physical Exorcism Series.

After Extra Case: My Girlfriend's Secrets, Lily becomes suspicious of Marty's increasingly possessive attitude towards Sally. Worse yet, Sally and Marty both mysteriously disappear at around the same time an innocent woman is beheaded in Footville, making Marty even more sus. At the same time, the modus operandi of the murder looks very similar to the serial murders committed by the infamous Cannibal Boy. In order to solve these mysteries, Lily will have to pay the Ghosteater duo to help her investigate Marty and the legend of the Cannibal Boy.

This visual novel contains examples of:

  • The Bad Guy Wins: In the epilogue, Marty succeeds in killing Lily as part of his insane plan to give Sally a new body. This was also due to Nya and Jade's manipulations, with both finding this tragedy entertaining.
  • Big Bad Ensemble: The Cannibal Boy of Mt. Candyhouse is unsealed and threatens to go on another serial killing rampage where they eat every part of their victims other than the head. Unfortunately, they ate Sally's body, leading to Marty becoming a copycat Serial Killer of women in an attempt to find a new body for Sally.
  • Collective Identity: The Cannibal Boy refers to both the conglomerate of evil spirits from Mt. Candyhouse and Brucie when he was a cannibalistic serial killer, since both eat every part of their victims except the head. Some books in the Hillpolly library note that the two Cannibal Boys had different frequencies for their murders.
  • Company Cross References: The Peacemaker Series, which is also developed by Sounding Stone and MaouCat, is a JRPG series in this setting that Sally and Lily are fans of, though this doesn't stop Nya from reincarnating Jade into the Peacemaker universe.
  • Connected All Along: The ending shows that Nya can reincarnate people into in-universe video games, and they reincarnate Jade into the world of the Peacemaker Series, revealing them to also be the Greater-Scope Villain of said series.
  • Cruel Twist Ending: The “Happy” Ending seems to be genuinely happy at first, since the Cannibal Boy is defeated and Sally is saved. Then the epilogue reveals that Marty really was the killer and was planning to kill Lily all along, which he promptly does, turning her spirit into the New Cannibal Boy.
  • Downer Ending:
    • In the normal ending, Lily delivers a Mercy Kill to Sally, but this causes her to become emotionally numb to everything except hating evil spirits, resulting in her becoming a ruthless Blood Knight exorcist. Marty is arrested for the various serial killings and is never seen again. In a twist, this is a somewhat happier outcome for Lily than the true ending.
    • In the true ending, it turns out that not all the victims were due to the Cannibal Boy. Some of them were actually Marty killing various women in order to find a new body for Sally, and Jade helps him kill Lily. Right before Marty beheads her, Lily swears to become an evil spirit to take revenge on Marty and Jade. Nya is pleased with Jade's work and "rewards" her by sending her to the world of the Peacemaker Series, which they are also presumably manipulating events in.
  • Flipping the Bird: The Hillpolly Library reveals that the middle finger gesture can stop the evil eye ability. In the final battle with the Cannibal Boy of Mt. Candyhouse, this proves to be more effective than Marty's low-quality Nazar.
  • No Periods, Period: Averted. Jade experiences a period, which she weaponizes with her telepathic ability to share pain.
  • Non-Standard Game Over: If Lily refuses to search for Sally, a mysterious man with Marty's silhoette will stab her after she leaves Silversmith. This is the real Marty, since the killer didn't use the Cannibal Boy's Modus Operandi of tapping on a window first and the killer isn't floating like most evil spirits. Additionally, Marty recognizes Lily when the Ghosteaters visit his house, showing that he was the one stalking her at the start of the game.
  • Rewatch Bonus: The game makes it ambiguous who the psycho killer from the fatal ending is. Later on, the games makes it seem like it's the Cannibal Boy disguised as Marty. However, the Cannibal Boy and most evil spirits are always floating while the killer isn't floating. This indicates the psycho killer actually is Marty.
  • Right for the Wrong Reasons: Lily, Jade, and Brucie initially suspect Marty is the Cannibal Boy and is behind a string of serial murders. It turns out he is responsible for killing two women, but he didn't eat them like the real Cannibal Boy. Instead, he attempted to attach their headless bodies to Sally's head, who was attacked by the real Cannibal Boy.
  • The Stinger: After the credits, a trailer is shown for Occult Club: Gathering 4 Fatal Stories, which appears to be a prequel showing Lily's days in the college's occult club. After that, Nya reincarnates Jade into the Peacemaker Series. The game then cuts to a trailer for Peacemaker: Bloody Emperor, with Jade being presented as one of the antagonists.
  • Unwitting Instigator of Doom: Marty forced Sally to come with him to bury Shadow's incriminating evidence on Mt. Candyhouse, but Sally accidentally broke the seal on the Cannibal Boy.
  • The Worf Effect: Even with the Nazar, which helped Marty defeat the enemies of Case 02: Paranormal Evil, he is unable to keep the Cannibal Boy from eating Sally's body below the neck. The next time he faces the Cannibal Boy, the latter breaks the Nazar. This is to show that the Cannibal Boy is much stronger than the evil spirits and undead from the hospital.
