  • Banned in China: On June 29, 1996, Duke Nukem 3D was put on the infamous German index by the BPjS.
  • Keep Circulating the Tapes: After the Duke IP changed hands to Gearbox, most digital releases of the game were pulled from storefronts.
  • Star-Making Role: To this day, the character of Duke Nukem is Jon St. John's signature voice-acting role.
  • Too Soon: Averted at least once. According to Word of God, a Shout-Out section to Oklahoma City bombing (which happened about a year before the release of the game) was made but deleted shortly before the game came out.
  • What Could Have Been:
    • 3D Realms considered doing a first person sidescroller game. Thankfully, this idea was scrapped.
    • There's also "lameduke", an early beta of the game. Thank your lucky stars that they decided to rethink the entire game concept. A map (Derelict) survived the purge, though.
    • Action Forms was going to release a urban hunting game called Duke Nukem: Endangered Species, a spinoff of 3D. It was scrapped.
    • The Pig Cop was originally supposed to be able to hit the player with their Shotgun, but 3D Realms dropped the idea, since it didn't work properly in the game. This concept was rescued later for Duke Nukem: Manhattan Project, which features the Pig Cops.
    • The developers planned many features that ended up being scrapped. Some of them are Dummied Out, and traces of them can be found in game data. Some planned features and concepts include laser sight, space suit, flashlight, multi-floor elevators, boss spaceship, scorpion-like tanks driven by Pig Cops etc.
    • As shown in old alpha and beta screenshots, the levels were originally more detailed than in the final game. A possible reason for the change is that the mappers had to simplify the design so that the game could be played on slower computers.
      • One notable exception was the level Derelict, which made it into the Atomic Edition (after they fixed the various bugs and glitches). It's a pretty elaborate level, despite being huge on par with The Abyss, the final level of Hollywood Holocaust.
    • Duke was originally intended to be able to use a space suit (those which you can see throughout the level Dark Side, the penultimate level of Lunar Apocalypse episode). This was meant to protect him later in the level where you actually have to go outside to the surface of the Moon. Not wearing a space suit would have resulted in an instant death after exiting the decompression chamber. It's unclear as to why they dropped the idea for the final release, but Duke being the Memetic Badass as he is, lack of ogygen and no atmosphere to protect him from the Sun's radiation is pretty much a normal day for him.
