E1M1: The Slipgate Complex, in the N64 version, as it appears in the 2021 remastered edition.
"While DOOM was ported to many platforms throughout its lifetime, Quake only saw two ports due to its much higher system requirements. One of those ports was to a 32-bit console, and the other was Quake 64. While both ports featured modified versions of the original levels in order to run on the hardware at the time, Quake 64 featured a few changes that differentiated itself from the original 1996 release. Colored lighting, a brand new soundtrack by acclaimed composer Aubrey Hodges, and two player split screen multiplayer were all unique to this version of the game. The unique nature of Quake 64 has caused it to be looked back on fondly by some players over the years, which is why we are happy to be able to bring it into modern times, with the original soundtrack intact."
Addons mod description in the 2021 remastered edition of Quake

Quake 64's single-player mode is a distilled version of the base game's single-player mode released in 1998 by Midway Games, later included as a downloadable addon to the game's 2021 remastered edition. Of the levels that were ported over, all of them were condensed into a single episode, making the whole experience more linear than usual.

The addon version of Quake provides an exact recreation of the game as presented on the Nintendo 64, complete with coloured lighting, a(n optional) platform-faithful CRT screen overlay graphical effect, and intact original soundtrack by Aubrey Hodges.

It contains the levels "The Slipgate Complex", "Castle of the Damned", "The Necropolis", "Gloom Keep", "The Door to Chthon", "The House of Chthon", "Ziggurat Vertigo", "The Ogre Citadel", "The Crypt of Decay", "The Wizard's Manse", "The Dismal Oubliette", "The Underearth", "Termination Central", "The Vaults of Zin", "The Tomb of Terror", "Satan's Dark Delight", "The Chambers of Torment", "The Haunted Halls", "The Tower of Despair", "The Elder God Shrine", "The Palace of Hate", "The Pain Maze", "Azure Agony", "The Nameless City", "Shub-Niggurath's Pit".

This version of the game and its 2021 remaster version show examples of:

  • Adapted Out: "The Grisly Grotto", "The Installation", "The Ebon Fortress", "The Wind Tunnels", "The Sewage System" and "Hell's Atrium" were left out from the game and the 2021 remaster.
  • Insistent Terminology: Per Nintendo's request, the game's loading screens have to say "Building" instead to make it seem like Nintendo 64 games don't have them.
  • Secret Level: Much like the source game, the available secret levels are "Ziggurat Vertigo", "The Underearth", "The Haunted Halls" and "The Nameless City".
  • Stylistic Suck: The mod version for the 2021 re-release displays the game as if it were on a CRT screen, with very low resolution included. This can be disabled with a specific console command or (after a later update) can be toggled within the re-release's display options.
  • Super Title 64 Advance: The official add-on has one, but not the actual Nintendo 64 version of the game.
  • Writing Around Trademarks: The official description of the mod as seen in the Updated Re-release's add-on menu refers to the Saturn as just "a 32-bit console" and is worded in a way so the Nintendo 64 console doesn't have to be mentioned.
