If you remember in the Mask of Life film, Takutanuva ressurrects Jaller from just his mask. The same mask was once worn by Lhikaan. So why wasn't Lhikaan ressurrected instead? Because he took the mask off before he died. Jaller was wearing the mask when he died, so he was essentially "saved" to the mask, allowing him to be ressurrected. Whether this is affected by the wearer's destiny as a toa (former or future) is anyone's guess.

The great beings built more than one Mata Nui, and each had it's own identical set of inhabitants. If the Matoran are considered specialized cells within Mata Nui's body and that the Red Star would reincarnate them rather than make a whole new replacement, this means that their individuality is important to the workings of Mata Nui, rather than being formed as a side effect of their creation. This also explains why we casually see so many universes go down the wrong path and in the end only our Mata Nui and Teridax face off; the others all failed in their task and died or wasn't fast enough because there was no conflict in them to speed things up. As for the one universe where Mata Nui was never built?
