* DeadpanSnarker: He tends to be very snarky in his making J.D. miserable or in response to things J.D. says. This dialogue from the second episode:
--> '''J.D.:''' How's it going?
--> '''Janitor:''' I'm 37 years old, and I'm a janitor; how do you think it's going?
--> '''J.D.:''' Now, there is nothing wrong with being a janitor.
--> '''Janitor:''' Really? Thank you. You...you've turned my life around. I'm gonna have to go tell my janitor wife and all our janitor kids that life is worth living; and that comes straight from our hero, Dr. Whoozitz.... Dr. Nothin'.

* PromotionToOpeningTitles: He becomes a regular in season 2, due to his BreakoutCharacter status.

to:

* PromotionToOpeningTitles: Subverted slightly. He becomes a regular gets promoted to the main cast in season 2, due Season 2 and, simultaneously, the theme for the first few episodes was expanded, allowing the Janitor to his BreakoutCharacter status.be present in them. However, a few episodes into the season, the intro was shortened back to its original length/style, meaning that he was no longer featured in them despite remaining a main cast member throughout the show's run.

Added DiffLines:

* SatelliteCharacter: In the first season he only exists to be JD's tormentor and never really interacts with anyone else. This changes starting in season 2, when he becomes a main character and has regular interactions with the rest of the cast, although he still torments JD at every chance he gets.

* {{Badass}}: He's easily the best doctor on the staff and has a wealth of experience and everyone fears him except for the Janitor.

* TheDreaded: Comedic example, but not even Cox and The Janitor have the guts to say anything bad about her. Even ''Kelso'' from the seasons, before he was softened up, avoided getting into a direct confrontation with her, especially when it pertained to work matters.

* {{Badass}}: He's frequently pulled pranks on Kelso and gotten away with it.

* ParentalSubstitute: To JD. Supposedly, he hates it. Although from [[AlternateCharacterInterpretation a certain point of view]] one wonders if he knows he's not the best role model for JD and is encouraging him to not see him as one.
** However, Dan (JD's older brother) confronts Dr. Cox at the end of the episode and pretty much says that despite the tough-guy act, Dr. Cox geuninely enjoys being looked up to by the newbies and being their idol, while also telling him to take that responsability seriously. Arguably, he really does enjoy getting to steer people on the right path.

At the same time, Dan points out that he really believes that under Cox's tough-guy act, he loves that the 'newbies' idolize him. --> '''Cox:''' (exasperated) "Look! We have me, emotionally-crippled narcissist. And you (JD's brother Dan), an emotionally-crippled narcissist who is in a bathtub filled with by now is mostly his own urine! Hopefully we can come together and make ONE fully functioning adult!"

Added DiffLines:

* {{Foil}}: At the same time, he is the polar opposite of JD in every way. Down to getting the unwanted attention that JD always sought from Dr. Cox. While he has little in the way of emotional needs, he also lacks the sense of empathy that makes JD such a gifted doctor.
