1* BigLippedAlligatorMoment: The fake ''Wolf Baby'' trailer. It's meant to be an homage to grindhouse trailers, but still doesn't have any real context in the movie itself.
2* CrossesTheLineTwice: Basically the entire movie.
3* CryForTheDevil: Attempted, but hardly ever any success is made. The killer had a miserable childhood that would make him sympathetic if it didn't come off as such a desperate attempt to justify his misogyny, coupled with the sheer pleasure he takes in his absurdly brutal murders.
4* DiagnosedByTheAudience: It's never made obvious what kind of mental diagnosis the killer has, but he spends most of the film mute, even in his childhood. And his mother and sister appear to think there's something wrong with him mentally. He does display a clear psychosis, as evidenced by his hallucinations of the Skull Boy. He also shows traits of sadistic personality disorder, seeing how he laughs as he mercilessly hacks his victims to pieces while they're alive.
5* EnsembleDarkhorse: While he's not exactly a minor character, being the guiding force behind the killer's murders and everything, the Skull Boy is often considered the most intriguing character in the film.
6* {{Narm}}: LOTS. It is, after all, a grindhouse homage. Namely, Shane Beasley's overacting when he's hacking into victims or raping their severed heads, as well as the scene towards the end where he hallucinates his mother's severed head calling him a "fucking freak" as he penetrates it.
7* NauseaFuel: There are many, ''many'' scenes in which the killer devours the eyeballs of his victims, and we get to see the whites of the eyes slowly ''ooze'' from his mouth as he bites down on them. The repetition doesn't make it any less gross. And that's not even mentioning that he likes to sexually penetrate the stumps of their necks.
8* NightmareFuel: It's a gory movie featuring a psychologically damaged serial killer with a fondness for cutting off heads and gouging eyes. This trope is guaranteed.
9** He has several bizarre dream sequences with [[EyesDoNotBelongThere a tree covered in eyes]]. And if that's not bizarre enough, he also fantasizes about having sex with a woman with no facial features except lips.
10** Some of the heads of his victims are shown on a shelf, rotting away. And that's not even mentioning when they appear to come alive at the end.
11** The entire climax of the film. [[spoiler:The killer slaughters Jess, then degenerates into total madness, ripping his own face off before locking himself up in his cage.]]
12* {{Squick}}: Besides the obvious gore, the sister is implied to have a warped, incestuous crush on her brother. This is first established when she asks if he's thirsty, and urinates on him.
13* TearJerker: As difficult as it is to feel any pity for the killer, the flashbacks to his childhood and his crying scenes can be rather heartbreaking.
14* TooBleakStoppedCaring: The movie's about a revolting SerialKiller who keeps having flashbacks to his abusive childhood. And none of the other minor characters are very likable, except for Jess and Maggot. So it's pretty easy to lose much care for what happens to anyone in the movie.
15* TheWoobie: Every single one of the killer's victims, with the exceptions of his mother and sister, and [[spoiler:Slick Vick.]] [[JerkassWoobie The killer himself as well]], to some degree.
