There, I think that should about do it. Massively cleaned up the original grammar and formatting, and also added a few things of my own into the mix. Anyone who sees any further improvements is free to add them.

It's "We wear the mask that grins and lies, It hides our cheeks and shades our eyes". Cleaning up the grammar of a poem isn't the best thing to do.
