This page needs to be fed into the main creature index. In fact, the work is mostly done, the modrons, mephits, rilmani, slaadi and yugoloths already have fleshed-out entries elsewhere.

I get why Planescape has a general Characters and Factions pages, because Sigil is a place partly defined by its characters and factions. But it doesn't need its own "Creatures and Races" page. I disagree with segregating monsters by origin setting to begin with, but beyond that, Planescape is less a distinct world like Eberron or Dark Sun, and more of an expansion pack that fleshes out D&D's default cosmology. Every creature on this page is, unless specified, assumed to exist no matter what particular campaign setting is being played - some details might change, but there are slaadi in Eberron, and the githyanki have visited Athas.

More to the point, these suckers have been showing up in the basic Monster Manual since 3rd Edition (which also included more obscure Planescape creatures like belkers, thoqqua, rasts and ravids). If we took this page seriously, moved all of the creatures that debuted in Planescape some 30 odd years ago here from the main index, the result would just be confusing omissions.

Removed from main page, as there is a cleanup effort on one of the pages linked here underway and I am not sure what to do with this entry:

This probably fits best under Dragon Rider. Rewrote slightly to mention that the githyanki ride young red dragons as mounts.
