Yeah, I tried to fix it by moving the picture up a notch but it's still there, weird.

Formatting with three exclamation marks at the beginning overlaps on pictures, changing it to two exclamation marks will fix it

At what point in the game is it stated that the Plague was made by the Outsider, and isn't the result of the Lord Regent's plans? I must have missed that.

I don't remember that either. I saw something on another page about how the Outsider enhanced the plague after it was brought to Dunwall, but I don't remember that in the game.
Does Emily become a Heroic Mime if chosen as protagonist in Dishonored 2?
