I haven't read "Wedding Vows" yet... It is described on the same site as "A tribute to marriage and idealised love with a vintage Americana feel," but its art style lacks any moe girls or bishounen.
I've been thinking about the virtual date game "Virtually Date Ariane" (which is Not Safe For Work!) lately. The dialogue, art, and setting are relatively realistic. I kinda wish that the game hadn't been created with polygon graphics, since the female character model can be Uncanny Valley-inducing.
If there really was an actually realistic Dating Sim, it would need to satisfy the following conditions: Almost every single girl you meet is already in a relationship. The ones that aren't are either lesbians, or they always travel in packs so that you can never approach just one of them. If you don't luck into an encounter with one of the girls at the exact moment she is breaking up with someone, you cannot win the game.
If there was a realistic reverse Dating Sim, that is, a dating sim where your character is a girl trying to date boys, it would need to satisfy the following conditions: Your character should stay reasonably thin to get a guy, although there are exceptions. Almost all options in conversations are successful, except for ignoring them or intentionally acting like a total psychopath. As long as you are not fat and you have not reached psychopath status, you can instantly win the game by mentioning anime and/or video games.

How about a game where you're an archaeologist with some ancient samples and need to date them using methods such as carbon dating (or using other radioactive isotropes) as well as your knowledge of history? That'd be a realistic dating sim...

You know, I think the Welcome To The NHK dating sim idea from a few pages (and therefore a few years) back actually has a lot going for it.
You could have it set up like a regular dating sim, where the main character is constantly bombarded by incredibly convenient and easy to pick up relationship opportunities, where it's usually easy to guess what the right option is... only whenever you choose those options, Satou just makes excuses to himself for not doing them, because he's too much of a sad sack to act like a dating sim protagonist. The whole game isn't a struggle between the player and the environment, but between the player and the main character.

If there are valid reasons why those pretty girls with(mostly) nice personalities are single, that is realistic but not ordinary. Of course the protagonist is also important. He should have a trait that makes all of these girls fall for him besides "He's such a nice guy." Determinator is a valid but overused trait. Even if his strength is determination, at least let the girls have some interaction with that guy before they fall in love ( not "I tried to jump high for hours and multiple girls fell in love with me").
My hatoful Monster Girlfriend is the President Ecstasy: Higurashi After in Summer (All ages memorial edition)I'd say that a realistic dating sim or romance based visual novel won't have lots of attractive single girls falling for the main character (and indeed, my understanding is that among actual dating sims, they usually don't.) Rather, it would have a bunch of characters who're prospects who the protagonist can encourage to fall for them with adequate effort.
Classic dating sims, unlike visual novels, tend to have a bunch of statistics and variables to manage, so getting any particular girl to fall for the protagonist requires significant time and effort (at least on the protagonist's part if not the player's,) and spreading that effort around tends to result in not getting very far with anyone.
In visual novels, where the difference between getting different routes may be dependent on a paltry few choices, writers tend to simplify matters by having several characters who're on the brink of falling in love with the protagonist regardless.
