Eh, sometimes I think as if I'm talking to someone else, or maybe an audience. Usually if I'm actually thinking about something I'll be talking about with someone else, but not necessarily. I think sometimes it happens because I interrupt my own thoughts to go down a tangent. Or maybe to just offer different perspectives/reactions, Idk.
Also I can switch which language I'm thinking on at will. And it's really hard, but if I try I think I can think about one or two things without thinking words or pictures?
