Like I said, she's A-san from the ghost story at the beginning of FHA. She used to live in Fuyuki until her neighbors went crazy and killed each other and their daughter. Ayaka could have helped the little girl, but she didn't answer the door since she apparently didn't know the kid was wounded and was kind of sick of dealing with her.
Edit: And I think her hair is dyed.

It should be obvious but I'll say it anyway: If you aren't following the flowchart, save before making a choice. FSN's reputation in regards to choices is actually undeserved since the narration and common sense usually point you to the proper choices (although some might not be completely obvious until you read the Bad End) but still. If you've been following the UBW anime, you can just use that for a guide to that route.

Yeah if this is true we have 2 FGO!Berserkers. Maybe we're going to get one set of new and old Servants for each era?

Strikes me as more Xerxes than Darius from the design. Did either of them go berserk or mad in their lives? I know Darius has been brought up here before on a wishlist as Rider. Need to see the clip to know for sure.

She's from Fuyuki. The whole Red Riding Hood Ghost rumor started was mentioned three years after the Grail War if I recall right.
Also I hope this meant Shirou was considered at one point, but he only had bad ends awaiting him.
