Error Index and Good Bad Translation use the same exact image. I noticed this when adding Error Index to image source.

Remove from Error Index. It's just a word relating to it, but doesn't illustrate anything on the index.

[up][up] If that could be turned into something relating to a game loading screen, that would work fine. As is, it doesn't seem to fit since that index is specifically for game errors.

I'd have suggested the Pac Man Kill Screen but... it's in use on Kill Screen.

3 is funny, but doesn't seem to illustrate the index at all. I would put a legit error message on.
