My President is Funny Valentine.The Mordheim PDF Power in the Stones has a D66* table for wyrdstone mutations, ranging from crystalline bodies to skull faces to clouds of flies. For one that isn't a random table, but could give you ideas for building one, Revlid's mutations fix for Exalted has an incredibly detailed listing of weird things that could happen.
Both are free.

The idea of percentiles (basically) with D6 is weird, but I like the table overall. Still looking at the Exalted one.

Black Crusade has what is mostly a copy of the original Realms of Chaos mutation table, on d100.
Then Tome of Corruption is better

(I have determined that this campaign will have both cosmetic and "effective" mutations, rolled separately.)

I need a table that can be randomly rolled for mutations. Not realistic mutations- totally out-there, potentially very powerful sci-fi/fantasy mutations. Anyone know where I could find such?
