I would generally put Mr. Fanservice under M, since "Mr" is an abbreviation rather than a little word like "A".

Yes, you only ignore the first word if:
- It's an article, and
- Adding or removing it does not significantly change the meaning of the phrase.

I would only ignore a personal title if the list was ordered by personal name (which means usually the surname). Trope names aren't personal names, even in cases where they're named after a person.

I was looking at the page for Dolphin Tale and couldn't help but notice that the Mr. Fanservice entry was between the Dueling Movies entry and the Feed by Example entry; in that case, (and I guess this extents to other formal titles like "Mrs." or "Dr.") should it go by the first letter of the formal title, or should it go by the first letter of the first word after the formal title?
