The fact that it's in the hands of a kid in front of a christmas tree establishes somebody thought it was for kids - the caption combined with that knowing smirk gives the "what do you mean it's not?".

While Pans Labyrinth is definitely a good example, I think it might be better still to use something that indicates the trope itself in the image. This◊ poster for Fritz The Cat does just that; showing one funny-looking animated cat groping another, with a tagline saying it's not X-rated for nothing.
