  • Awesome Music: "Father", the hauntingly beautiful theme for the first meeting with Noah. Ironic, as he's actually the villain of the story.
  • Captain Obvious Reveal: Two major twists in the story have been guessed by pretty much every player:
    • Caroline actually being one of the missing twins is a theory that most would gravitate towards from the initial setup, as she's drawn to the island by a photo of the twins igniting migraines that are very blatantly her missing memories beginning to return. Somewhat less obvious until near the end is that the bloated mass that chases you for much of the second half of the game is the other twin.
    • Father Noah's reveal as the Big Bad orchestrating the events in the present by virtue of him being the only other human NPC in the game (William is killed shortly after Caroline is knocked out at the beginning of the game). The only other alternatives would be a Stranger Behind the Mask or the game having no Big Bad at all, despite the questionable circumstances that brought Caroline to the island.
  • Complete Monster: Father Noah Morissette is the leader of the Pollux Sect. Yearning for a greater purpose in life, Noah willingly joined the Sect after he was unable to sacrifice himself to help resurrect their god. Working alongside the founder, Bertran Wildberger, Noah assisted him with human experimentations and sacrificing various twin children in hopes of bringing about their god so he could reshape the world in his image. When Bertran is unable to sacrifice his own grandchildren, Emma and Anna, and tries to rescue them, Noah has Bertran killed and kidnaps Anna after she tries to escape. He later keeps her locked away in a cell underground until she mutates and loses her sanity, all while continuing his schemes. Upon finding out that Emma was still alive and living under a new name, Caroline Walker, Noah lured her to the Wildberger Hospital so he could use her to unlock the chamber to the Sect's dormant deity. When his plans of kidnapping and sacrificing Caroline fail, he uses his own blood to awaken the deity in an attempt to wreak havoc on the world.
  • Goddamned Boss: The final boss arena is a series of platforms and walkways. You beat the boss by interacting with devices on the platforms, while dodging the boss' spitting attacks. Most of the difficulty comes from the constant changes in viewpoint as you move around: these make it difficult to locate the platforms, especially when you can't stop to gain your bearings.
  • Scrappy Mechanic:
    • The in-game map was criticized after initial release for not showing your current location, or if a room has been fully cleared of collectibles or not, such as in the remake of Resident Evil. While an update added after release makes the room you're currently in glow on the map, there's still no way to tell if you've missed something in any given room.
    • Criticism has been levelled at the inconsistent ammo pickups. Some nail clips will give you ten or more nails, some will give you just one.
  • That One Puzzle:
    • Most of the puzzles in the game are pretty straightforward after you have had the "Aha!" moment. The pipe puzzle near the end, however, is just a confusing tangle of pipes and trial-and-error with no underlying principle to guide you.
    • You open one door by knocking on it in the right rhythm. Exactly the right rhythm — it is possible to knock several times and not notice the difference between the knock that worked and the knocks that didn't.
  • Unintentional Uncanny Valley: Some of the face and body models are a bit off: not deliberately deformed or stylised, just not quite right. For example: when Caroline gets out of the bath, her body is probably meant to look wet, but instead it looks varnished.
  • WTH, Costuming Department?: A common criticism of Caroline's design is that it looks impractical and overtly sexualized. It's hardly what you'd call Stripperiffic, but still an odd choice to voluntarily investigate a mysterious and possibly dangerous Abandoned Hospital while dressed like a model ready for a gothic horror photoshoot. Later, it was averted with an update that introduced an optional second outfit, composed of a jacket, jeans and boots which is far more practical and realistic, available from the start and accessible from the menu.
